U.S. Army First Lieutenant Mathew Gregory Fazzari, 25, of Walla Walla, Washington, was killed in a helicopter crash in Ghazni province, Afghanistan, on June 6, 2012.

Their Story

First Lieutenant Mathew Gregory Fazzari was a 25-year-old Army officer from Walla Walla, Washington. He served as a member of Troop F, 1st Squadron, 17th Cavalry Regiment, 82nd Combat Aviation Brigade, based at Fort Bragg, North Carolina.

On June 6, 2012, Fazzari was killed when his OH-58D Kiowa Warrior helicopter crashed in the Ghazni province of eastern Afghanistan. The U.S. Department of Defense stated the crash was the result of a hostile incident. Another soldier, Chief Warrant Officer 2 Brian D. Hornsby, 37, of Melbourne, Florida, also died in the crash.

Fazzari's death occurred during Operation Enduring Freedom, the U.S.-led military campaign in Afghanistan that began in October 2001. By mid-2012, the conflict was in its eleventh year, with coalition forces engaged in counterinsurgency operations and training Afghan national security forces.

According to a 2012 report from the Army Times, Fazzari was posthumously promoted from the rank of second lieutenant to first lieutenant. He was survived by his wife, parents, and siblings.

First Lieutenant Fazzari was one of 2,356 American service members who died during Operation Enduring Freedom. His name is inscribed on the Afghanistan War Memorial at the Walla Walla Veterans Home.
